Please provide a Start and End Date for your Clinic. If the Clinic is only a single day, provide the same date for Start and End. If your Clinic is multiple days or weekends, please provide the date of your first Clinic and the date of your last Clinic. In the case of a multi day Clinic, use the Clinic Description (above) to provide a full date list for participants.
