Co-Ed Waiver Application

A co-ed waiver should be submitted for athletes who are wishing to participate on a team opposite to their gender. For example, a junior male athlete wishing to participate on a junior female team OR a junior female athlete wishing to participate on a junior male team.

Please be aware that a male player must be at least two age groups below the age division the team is currently playing in. (e.g. a 14 year old male can play Girls 16 and up).

Please complete the following application entirely. If this application is approved, it will only apply to the following NCVA Sanctioned Events:

Power, Premier, Non-League, and Youth League
California Kickoff & Kickstart
President’s Day Tournament
Golden State Classic Tournament
March Madness
Far Westerns Tournament (No Dinx Division ONLY)
Bay View Classic
Boys’ Summer Kickoff

You may not use this waiver for any National Qualifier bid divisions or the USAV Junior National Championship. This waiver is for Northern California Region-Approved events ONLY.

Please Note: NCVA and/or Tournament Staff has the authority to remove the player from the match and/or roster in response to sufficient, valid complaints.
